What do I need to provide for someone to collect documents for me?
A signed patient authorization letter naming the collector, the patient's government ID copy, and the collector's own ID. We provide the exact letter format — you sign, photograph, and WhatsApp it back.

Do I need a Power of Attorney to collect medical records?
No — a simple signed authorization letter is sufficient for most records. A notarized POA is only needed for sensitive documents (MLC, some indoor files) or when the patient can't sign.

What if the patient can't sign the authorization?
Hospitals generally accept next-of-kin authorization — spouse, parent, or adult child — with relation proof. Some sensitive records may need a notarized declaration.
